The Forms tab on the client dashboard shows all form submissions from a specific client. You can review responses, filter by form or date, mark submissions as reviewed, and add internal notes — making it easy to stay on top of client check-ins and feedback.
Accessing Client Form Submissions
Go to Trainerize Clients.
Click on a client's name to open their Client Dashboard.
Select the Forms tab at the top of the dashboard.
All form submissions from this client are displayed in a table, sorted by submission date with the newest first.
Note: The Forms feature requires a Starter or Business plan. If you are on the Essential plan, you will see an upgrade prompt.
What You Can See
Each form submission row shows:
Form name — Which form the client submitted.
Submission date — When the client submitted the form, displayed in your business timezone.
Report week — The week the submission is assigned to (if applicable). Late submissions are flagged.
Review status — Whether the submission has been reviewed by a team member, and by whom.
Viewing a Submission
Click the View button (eye icon) on any submission row to open a detailed view. This shows:
All questions and the client's answers, displayed in the original form order.
File attachments — If the client uploaded files (images, PDFs, or other documents), you can preview or download them directly.
Rating questions — Displayed with the appropriate scale and labels.
Contact details — If the form collected contact information, each field (name, email, phone, address) is shown separately.
Filtering Submissions
Use the filter controls at the top of the submissions table to narrow your view:
Filter by form — Select a specific form from the dropdown to see only submissions for that form.
Filter by date range — Set a start and/or end date to see submissions within a specific time period.
Filter by review status — Choose "Reviewed", "Not Reviewed", or "All" to quickly find submissions that still need your attention.
Click Clear filters to reset all filters at once.
Marking Submissions as Reviewed
Tracking which submissions you have reviewed helps you stay organized, especially when managing many clients.
Click the review button (circle icon) next to any submission.
The submission is marked as reviewed with your name and a timestamp.
To undo, click the review button again to remove the review status.
The reviewer's name is displayed so that teams with multiple coaches can see who has already reviewed each submission.
Internal Notes
You can add internal notes to any form submission. These notes are only visible to your team — clients cannot see them. Use internal notes to record observations, flag concerns, or leave context for other coaches on your team.
Reassigning Report Weeks (Late Submissions)
If a client submitted a form late and it was assigned to the wrong report week, you can reassign it. This option only appears for late or outdated submissions.
Click the reassign week option on the late submission.
Select the correct date. The system automatically calculates the correct Monday (week start).
Click confirm to move the submission to the new week.
This keeps your weekly reports accurate, even when clients submit late.
