How to Set Up Form Submission Notifications (Simple, AI-Powered & Custom)

Configure what message clients receive after submitting an AI Form — choose Simple confirmation, AI-Powered personalized feedback, or Custom with your own text.

When a client submits an AI Form, you can automatically send them a confirmation message. Choose from 3 notification types depending on how personalized you want the response to be.


How to Configure Notifications

  1. Go to AI Forms

  2. Open the form you want to configure

  3. Click the Settings icon (gear) in the form builder

  4. Toggle Send notification after submission ON

  5. Select your Notification Type


Notification Type 1: Simple

A basic confirmation message that lets the client know their form was received.

What the client receives:

"Hey [Name], we just received your submission for [Form Title]. A team member will get back to you shortly.

View your submission: [Link]"

The link to view their submission is always included automatically (shown by the "Link included" badge).

Best for: Basic acknowledgment — onboarding forms, check-in forms, agreement forms.


Notification Type 2: AI-Powered

The AI reads the client's form responses and generates a personalized message that references their specific answers.

How it works:

  1. Client submits the form

  2. AI analyzes all their responses

  3. AI generates a warm, personalized 2-3 sentence acknowledgment that references 1-2 specific answers

  4. The message is sent with a link to view their submission

Example: If a client answers "Goal: Weight Loss" and "Training days: 4 days/week", the AI might say: "Thanks for sharing your goals, Sarah! We can see you're focused on weight loss and planning to train 4 days a week — that's a great starting point. A team member will review your answers and be in touch shortly."

Best for: Onboarding forms, assessment forms — anywhere you want clients to feel heard immediately.


Notification Type 3: Custom

Write your own message with full control over the text.

Available placeholders:

  • {name} — the client's first name

  • {form} — the form title

Example: "Thanks {name}! Your {form} has been received. We'll have your program ready within 24 hours."

You can also toggle Include link to view submission on or off. When enabled, a link is appended so clients can view and comment on their submission.

Best for: When you want a specific branded message, CTA, or next-step instructions.


AI Feedback Template

Below the notification type, there's an AI Feedback Template dropdown. This controls which AI template is used when generating detailed feedback for form submissions (used in AI Check-Ins and form review, separate from the notification message).

Options:

  • No specific template (use business default) — uses your default template

  • Any custom AI template you've created


How Notifications Are Delivered

Notifications are sent as a Trainerize message in the client's chat thread. If the client has Telegram configured as their preferred channel, the notification is sent via Telegram instead.
