One of the most powerful use cases of AI Check-Ins is automating the delivery of forms to your clients—without needing to ask for names or emails.
This means:
📩 Forms are personalized
🧠 Submissions are automatically linked to the correct Trainerize client
📅 Responses are organized by the correct week
📊 All results show up inside the client’s compliance tracker
✅ Step-by-Step: How to Add Forms Inside a Check-In Template
Go to AI → Check-Ins → Templates
Create or edit a check-in template
Click “Insert Placeholder”
Select the Forms category
Choose from the forms you’ve already created
Once added, the placeholder will insert a personalized link like this (only clients can see as soon as you send it to them):
https://app.fitmetrics.ai/form/e5fecc49-880e-464e-8a3f-e1e0705bf7fe?id=5208028&week=2025-08-11
✅ The link will dynamically update with:
The Trainerize Client ID (id=...)
The Date (week=...)
🔒 Why This Is Important
No More Asking for Name/Email
The system already knows who the client is. The link auto-attaches their Trainerize ID.
Auto-Attached to Week
When sending the form via check-ins, the URL will include the date. For example if date is 14th of August 2025 then it will be attached on week 11th to 17th of August 2025 —this helps properly attach the form submission to the correct week in the compliance tracker.
📆 How the Week Logic Works
The system currently defaults to “Attach to the current week”.
🔁 Example:
If today is Sunday and you send the check-in → Form will be attached to this week’s data.
If you send it on Monday → Still attached to the current week (not previous).
(More logic options are coming soon.)
💡 Tip: Use AI Check-Ins to Send Forms at Scale
This is the easiest way to:
Send a Weekly Check-In Form
Collect Fitness Assessments
Automate Feedback Surveys
… without ever needing to manage links or contact info manually.
And most importantly:
📥 All submissions are stored under the client’s record, so you always know who submitted what, and when.
