Pages

Forms can be:

  • Single page – Quick forms with 3–5 questions

  • Multi-page – Best for onboarding, assessments, long questionnaires

When to use single page:

CHOICE ELEMENTS


Single Choice (Radio Buttons)

Choose ONE option.

Best for:

  • Yes/No

  • One-answer questions

  • Goal selection

  • Status selection

Advanced Features for Single Choice Element only:

1. Auto-Advance

Automatically moves to next page when option selected.

Use when:

  • Only one question on the page

  • You want fast navigation

Not recommended when:

  • Other questions exist on same page

2. Skip Logic

Conditional page navigation.

Examples:

  • "Do you have any injuries?"

    • Yes → Injury assessment page

    • No → Next normal page

Each option can jump to a different page. For this to work you need to make sure there are multiple pages on your form.

3. Field Mapping

Map choices to:

  • Goal (Weight Loss, Muscle Building, etc.)

  • Compliance Status (Active, Vacation, Injury, Sick, etc.)


Multiple Choice (Checkboxes)

Choose multiple options.

Best for:

  • Available training days

  • Equipment owned

  • Preferences

  • Dietary restrictions


Choice Grid (New)

A matrix/grid where each row has the same set of single-choice column options.

Best for:

  • Rating multiple items on the same scale (e.g., "Rate your energy, sleep, stress on a scale of 1-5")

  • Comparing preferences across categories

  • Likert-scale surveys


Checkbox Grid (New)

A matrix/grid where each row allows selecting multiple column options (checkboxes instead of radio buttons).

Best for:

  • Selecting multiple attributes per category (e.g., "Which supplements do you take on each day?")

  • Availability scheduling (days vs. time slots)

  • Multi-select comparisons


Ranking

Drag items into order.

Best for:

How to Build a Form

1. Create the form

Go to AI Forms → Create New Form → Name your form.

2. Add pages

Use 3–7 pages for longer forms.

3. Add elements

Drag any of the 20 elements into the page.

4. Configure elements

  • Write clear questions

  • Add mapping

  • Add skip logic

5. Preview

Use Preview to test navigation, skip logic, page flow.

6. Publish

Yes — newest submission replaces old values.

Not supported yet.
