FitMetrics Forms are more than just a replacement for Typeform or Jotform—they’re fully integrated, dynamic, and built with fitness coaches and health professionals in mind. Whether you’re creating onboarding forms, feedback surveys, or medical history assessments, you have a wide toolbox of elements at your fingertips.

🧩 3. Form Elements You Can Use

Here’s a breakdown of the form components you can use to craft any experience:

🧠 Basic Elements

  • Heading Use this to organize your form into sections or titles. Ideal for longer forms like assessments.

  • Paragraph Add context, explanations, disclaimers, or instructions between sections.

  • Short Answer Great for one-liner responses like “What’s your current weight?” or “Your coach’s name?”

  • Long Answer Use for open-ended questions like “Tell us about your current diet” or “What are your main struggles?”

✅ Selection-Based Elements

  • Single Choice Works like radio buttons. Best when users should only select one answer.

  • Multiple Choice Allows users to choose more than one. Ideal for questions like “What are your goals?”

  • Star Rating Quick 1–5 rating scale. Good for satisfaction surveys or self-assessments.

  • Slider Ask users to slide between a number range. Example: “Rate your stress from 1 to 10.”

  • Opinion Scale A Likert-style scale, perfect for collecting feedback like “Strongly Disagree” to “Strongly Agree.”

  • Ranking Users drag and drop items in order of priority. Example: “Rank your top health goals.”

📇 Client Info Capture

  • Contact Details Automatically request name, email, and phone in one section.

  • E-Signature Great for agreement forms or legal disclaimers—your client signs digitally.

📎 Media Upload & Embeds

  • Image Upload Ask clients to upload gym photos, transformation pics, or meal snapshots.

  • File Upload Great for uploading lab test results, doctor’s notes, PDFs, or meal plans.

  • HTML Embed Add widgets like:

    • Calendars (e.g. Cal.com or Calendly)

    • Videos from YouTube or Wistia

    • Custom HTML widgets (surveys, timers, etc.)

  • Divider A simple horizontal line to break up sections and make forms more readable.

🔄 4. Advanced Logic Support

To make your forms smart, responsive, and personalized, FitMetrics includes powerful logic features.

✅ Conditional Logic

Create dynamic forms that change based on a user’s input. For example:

  • If someone selects “Yes” for “Do you have any injuries?”, a follow-up long-answer field can appear asking for more details.

  • If a client chooses “Vegan” in a diet-type question, hide questions related to animal-based foods.

Supported logic includes:

  • Show/Hide elements based on Single Choice or Multiple Choice answers

  • Trigger sections to appear only under specific conditions

  • Combine conditions using AND / OR logic

  • Create branching experiences to keep forms short, relevant, and engaging

⚙️ Embed Advanced Tools

Using the HTML Embed field, you can extend forms further:

  • Add a Cal.com calendar so clients can book a call right after submitting.

  • Embed a Wistia video to educate clients before answering.

  • Include a terms and conditions page or privacy policy link.

💡 Pro Tip: Less is More

Use conditional logic to only show what matters, keeping the form clean and improving completion rates.
